Mail Service Pharmacy CSCD 5 - Disclosure Verification

CSCD 5, a partner to CSCD 4, gives payers the right to verify the disclosures required in CSCD 4, pursuant to the contract between the payer and the organization.

We recommend using the same documentation as you used in CSCD 4:

1. a P&P that describes in detail your disclosure-to-payers methodology, and  
2. a template payer/client agreement that demonstrates your compliance with your disclosure policy. 

Again, make sure everybody involved understands that this standard (and your corresponding P&P) applies to disclosures to your organizational payer-clients, not to consumers. 

No need for a new P&P here -- just use the one you submitted for CSCD 5, making sure it has an audit provision in it.
