The Basics

This standard describe what is required in your notification that you have approved a request for continued hospitalization or other ongoing services:

• Number of extended days or units of service
• Next anticipated review point
• New total number of days or units of service that you've approved
• Date of admission or beginning of services

This notification may be oral or written, but you must document it in the case file in either event.

Management Tips

Develop a template for notification -- either a letter or a script that can be documented in the case file.  If your concurrent review nurses are doing a lot of such reviews each day, help them out by creating a log on which they can record all the information required by this standard.  If you are managing this process through an electronic platform, it will make things much easier if you build in a letter-generation feature.

For the desktop review, submit the P&P and any templates for notification.  

During onsite review, the reviewer will confirm implementation of your P&Ps by reviewing case files and interviewing staff members.
